Can I complete the survey on my mobile device (iPad/iPhone/Android/Windows Phone)?
Yes, you can use any mobile device, laptop or computer that has a connection to the internet.
Do I have to complete the entire survey all at one time?
No, you can log out and log back in as many times as you need to. You will have to use the link in the email you received, so do not lose that. If you have any trouble, email us at Surveycenter@inclusiondx.com.
Is our identity protected? How can I be sure that no one will know my answers?
This survey is completely confidential. We have multiple safeguards in place to ensure that your individual responses will never be linked to your name or other identifying information.

The online survey is being hosted on an external secure website. This is outside and unconnected to any company or organization’s intranet and computer network.

Answers are never linked to your name or identifying information.

No one will ever able to link your answers to you personally.

We will summarize the results in a statistical format, making it impossible to identify the answers of a specific individual. Any write-in answers will be handled carefully and only shared in reports IF possible to do so in a way that insures complete anonymity.

We do not provide the information we get from this survey to any one, any company or organization, at any time, for any reason.

We will report the survey results in aggregate to your company. There are no names or email addresses in the data file with the survey responses. No one can get access to the dataset and try to piece together who you are. We never give the raw survey dataset to any employer or company. Our employees are bound by strict confidentiality and non-disclosure rules.
WE have conducted dozens of studies and assessments with hundreds of thousands of participants. We have never had a confidentiality breach. We stake our reputation on it.
Can I refuse?
Yes, of course. This is completely voluntary. We hope you will participate – but no one will know if you participate or not. We use an automated reminder system so no one knows who participates and who does not. For that reason you may continue to get automated reminders. Just ignore them.
Can I answer some questions or not others?
You can skip any question or any part of the questionnaire and just answer the questions you want to. Of course, we can paint a more complete picture if we get complete feedback from everyone, but it is entirely your choice.
Why are you asking about my race/ethnicity, sexual orientation, gender identity, whether I have a disability, and/or about my BMI?
In some companies, employees who are racial and ethnicity minorities, LGBT, have disabilities or obesity size have different experiences than others. Your company wants to make sure that all employees are treated with equal respect, opportunities and dignity. We will compare the responses of employees in any of these groups to those who are not (for example employees who have a disability to those who do not) to see if they are, on average, having different work experiences).
How will you protect confidentiality when you report study results? Suppose I am in a very small group? Will my supervisor see my write-in responses?
Our first and primary guiding principle for reporting is:
Do no harm.
We report on subgroup experiences when we can do so in a way that provides insight without compromising employee confidentiality.
Here is an example of something we would not include in a report: Five or less employees identify as LGBT (or another minority group) and answers questions about their experiences as an LGBT employee (or other minority group) – we would not provide this subgroups responses because it would be possible to guess the identity of the individual. Responses of LGBT (or other minority groups) will be combined together such that individual employees cannot be identified.
If you provide a write-in responses we will delete any information that identifies a specific individual or that might put you at risk for being identified.
